A 36 y/o male presented with multiple sinus infections over the previous six months and no relief from antibiotics. He was first diagnosed with chronic sinusitis, worsening persistent allergic rhinitis, and nasal polyposis. Advair, Nasonex, Astelin and Singulair had been initially prescribed. A 12 day course of prednisone was prescribed to alleviate symptoms.
Xoran MiniCAT CT was performed in June 2007. It revealed severe inflammation throughout all of sinuses. Nasal passages were severely congested, with acute and chronic inflammatory exudates. There was a question of bony invasion (Figures).
The patient was immediately referred to a local ENT. The ENT specialist was concerned about the possibility of an expancile mucocele in the right. frontal sinus, with complicating erosion of bone towards the anterior cranial vault, as well as potentially along the medial aspect of the right orbit. The patient was then referred to the University of Colorado Hospital for surgery. A UCH surgeon performed a polypectomy and decompression of the right mucocele endoscopically. The patient had an uneventful post-operative course and is feeling well. The MiniCAT sinus CT helped diagnose this patient's mucocele.
